Program Intensive Outpatient Program Monday 6:00pm to 9:00pm Wednesday 6:00pm to 9:00pm Friday 6:00pm to 9:00pm The Intensive Outpatient Program is designed specifically for individuals who need additional support such as drug and alcohol group therapy while in a halfway house setting or sober living environment. Some of the group topics covered in the Intensive Outpatient Program; relapse prevention, coping with stress, family conflict, external triggers and internal triggers, dealing with feelings and anger management. Program Relapse Prevention Track Integrity Way and Transformations Treatment Center also offer a two week Relapse Prevention Track for individuals that relapsed while in a halfway house or sober living environment. Many halfway houses refer clients to the Relapse Prevention Track should they test positive for drugs or alcohol. Once they have completed the two week Relapse Prevention Track they may return to the halfway house or sober living environment. During the Relapse Prevention Track, clients will explore why they used drugs or alcohol, what lead up to the relapse, how they change the negative behavior in the future, and what preventative measures need to be in place to remain clean and sober once they return to the halfway house or sober living environment. Transformations Treatment Center is a 30,60,90 day Intensive Outpatient Program. Our three phase step down program is designed to help our clients successfully transition back into society. The appropriate Phase will be determined during the initial screening. Our clients that are in need of supportive housing can reside in the Integrity Way Transitional Housing program. Single rooms with queen size beds have been made available for Transformations Treatment Center clients. The following programs are included in Phase I during the 30 day period.
Phase II is designed for those discharging from an Inpatient facility and or those who are in need of extensive Life Skills Coaching. The following programs are included in Phase II during the 30 day period.
Phase III
is designed for those who are currently employed or are
attending school. Employment must be approved by
staff. Anyone entering Phase III will have a minimum of
60 days clean and sober.
Group Topics:
Transformations also offers a faith based program as an alternative to our traditional treatment program. Transformations Treatment Center provides a court liason for individuals who have pending legal issues. Integrity
Way Transitional Housing and Transformations Treatment Center are two
separate organizations working together to better serve those addicted
to drugs and alcohol.
